Subchapter 6 - APPLICATIONS AND PETITIONS GENERALLY

All applications and petitions shall:
(1) State clearly and concisely the authorization or relief sought;
(2) Cite the appropriate statutory provision or other authority under which commission authorization or relief is sought; and
(3) In addition to specific requirements for particular types of applications (see subchapters 7 to 10), state the following:
(A) The applicant's legal name and location
of principal place of business, and, if a corporation, trust, association, or
other organization, the state under whose laws the applicant was
organized;
(B) The name, title, and
address of the person to whom correspondence or communications in regard to the
application are to be addressed. Notices, orders, and other documents shall be
served upon the person named, and that service shall be deemed to be service
upon the applicant; and
(C) If ex
parte action or relief pending full hearing is sought, the necessity or
emergency justifying the requested action.
