(a) All credit insurance shall be evidenced by an individual policy, or, in the case of group insurance, by a certificate of insurance. The individual policy or certificate of insurance shall be delivered to the debtor in accordance with section 431:10B-107(a), HRS.
(b) Each individual policy or certificate of insurance shall set forth such information as is required by sections 431:10B-107(b), 431:10B-109(b), and any other appropriate sections of the HRS.
