Subchapter 2 - ELIGIBILITY AND ENROLLMENT
Section 14-51-12 - Enrollment

(a) An employee may enroll in this premium conversion plan by completing a benefit election form(s) in accordance with subchapter 3, benefit elections, or by any other process authorized by the plan administrator.

(b) While an employee is a participant in this plan, any required employee monthly contributions for eligible expenses will be paid by the employer.

(c) Enrollment in this plan shall constitute an employee's election to include all eligible expenses for all component plans the employee is enrolled in.
