Section 12-21-5 - Notification

(a) Every employer shall, at the time of each payment of wages, furnish each employee an itemized statement in writing showing:

(1) The name of the employer;

(2) The name of the employee;

(3) The inclusive dates of the period for which the employee is paid;

(4) Total gross compensation;

(5) The amount and purpose of each deduction, except that all deductions made on written orders of the employee may be aggregated and shown as one item;

(6) Total net pay; and

(7) Date of payment.

(b) The pay statement shall be furnished in a form that the employee may retain as a personal record. It may be in the form of a section detachable from the check or a separate form.
