If an insurance company refuses to cover any employer-applicant, the insurance company shall notify the employer-applicant in writing of the name and address of the pool manager of the Hawaii temporary disability insurance risk spreading plan. A copy of the letter shall be kept in a separate file at the insurance company for the department's reference.
