Subchapter 2 - REQUIREMENTS FOR CERTIFICATION
Section 11-177.1-10 - Certified substance abuse counselor - licensed physician (specialty)

(a) The applicant shall be a physician certified by the American Society of Addiction Medicine or board certified in addiction psychiatry by the American Board of Psychiatry and Neurology.

(b) The applicant shall submit to the department:

(1) A completed application for certification;

(2) A copy of the applicant's current certificate;

(3) A copy of the applicant's current license to practice medicine in the State of Hawaii; and

(4) The appropriate application fee pursuant to subchapter 4.

(c) Individuals may apply for recertification up to sixty days prior to the expiration date of their current certification. Applicants for recertification shall provide the following documents to the department:

(1) A completed application for recertification;

(2) A copy of the applicant's certification, as described in subsection (b)(2), and the applicant's current license to practice in Hawaii, as described in subsection (b)(3).

(d) Individuals certified pursuant to this section are not eligible for reciprocity.
