A person required to file a statement pursuant to O.C.G.A. Section 33-13-3shall furnish the required information on Form A, hereby made a part of this Regulation. Such person shall also furnish the required information on Form E, hereby made part of this Regulation and described in Rule 120-2-23-.12.
