Section 69I-10.081 - Duplicate Warrants

When a warrant has been lost, stolen, or otherwise cannot be located, and the payee is entitled to the proceeds, it is necessary for the agency to request that the Chief Financial Officer place a stop order on the warrant in order to obtain a duplicate. This is followed up by the agency with an Affidavit for Duplicate Warrant (Form DFS-A1-408) signed by the payee and notarized, or signed by an authorized agency employee if the warrant was never delivered to the payee. When it has been determined that the original warrant has not been paid, a new warrant bearing the exact information as the original is forwarded to the agency for delivery to the payee. Form DFS-A1-408 (revised 4/99) is hereby incorporated by reference and is available from the Department of Financial Services, Reconciliation Section, 200 East Gaines Street, Tallahassee, Florida 32399-0354. The original warrant should not be cancelled when a duplicate has been issued. In the event that the original warrant is found, the agency is requested to return the original to the Chief Financial Officer to be voided.
