Section 69B-221.060 - Notice of Change of Information

(1) A bail bond agency licensee must accurately maintain all information within its MyProfile "Bail Bond Agency Profile" within the Department's online portal located at https://dice.fldfs.com/public/pb_index.aspx.

(a) Any change to the agency information must be reported to the Department using the licensee's MyProfile account within the Department's online portal within 10 working days of such change using Form DFS-H2-1564, Notice of Change of Information, which is incorporated by reference in Rule 69B-221.155, F.A.C.

(b) The information which must be kept accurate includes:
1. Agency name;

2. Owner or officer(s);

3. Business locations;

4. Business address;

5. Mailing address;

6. Email address; and

7. Work phone number.

(c) The licensee is responsible for notifying the insurer, managing general agent, and clerk of each court in which the licensee is registered of any change reported to the Department within the same period under this subsection.

(2) A bail bond agent licensee must accurately maintain all information within its MyProfile individual account "Licensee Profile" within the Department's online portal located at https://dice.fldfs.com/public/pb_index.aspx.

(a) Any change to the individual's information must be reported to the Department using the licensee's MyProfile account within the Department's online portal within 10 working days of such change using Form DFS-H2-1564, Notice of Change of Information, which is incorporated by reference in Rule 69B-221.155, F.A.C.

(b) The information which must be kept accurate includes:
1. Name;

2. Email;

3. Home address;

4. Business address;

5. Mailing address; and

6. Home and work phone number.

(c) The licensee is responsible for notifying the insurer, managing general agent, and clerk of each court in which the licensee is registered of any change reported to the Department within the same period under this subsection.
