Chapter 64D-3 - CONTROL OF COMMUNICABLE DISEASES AND CONDITIONS WHICH MAY SIGNIFICANTLY AFFECT PUBLIC HEALTH
Section 64D-3.034 - Cancer Reporting

(1) Reporting Requirements:
(a) Each facility and laboratory
licensed under Chapters 395 and 483, and Section
408.07(20),
F.S., respectively and practitioners licensed under Chapters 458, 459, 464,
F.S., are required to report to the Florida Cancer Data System as required by
Section 385.202, F.S., within six (6)
months of each diagnosis and within six (6) months of the date of each
treatment.
(b) Each facility shall
submit each cancer case report electronically. Those facilities with fewer than
35 cancers annually requiring abstracting may submit paper copies or portions
of the medical record, provided the copies contain all of the required
information as per paragraph (1)(c).
(c) The data items, coding schemes,
definitions, record layouts and reporting procedures are to follow the guidance
provided in the Florida Cancer Data System Data Acquisition Manual (2005, or
current year edition), incorporated by reference, available at:
http://fcds.med.miami.edu/inc/downloads.shtml.
(2) Not withstanding subsection (1), each facility, center and laboratory that reports cancer cases to the Florida Cancer Data System shall make its records available for on-site review by the Department or its authorized representatives.
