An applicant for certification by endorsement shall submit a completed Certified Nursing Assistant Licensure by Endorsement Application, DH-MQA 5022, 06/20, incorporated herein by reference, and may be obtained from https://www.flrules.org/Gateway/reference.asp?No=Ref-12204, from the Board office or on the Board's website: http://www.FloridasNursing.gov.
