Section 64B5-2.0142 - Application for Health Access Dental License

Any person wishing to be issued a Health Access Dental
License shall apply to the Board of Dentistry. The application shall be made on
the Application for Health Access Dental License form #DH-MQA 1154 (Rev.
08/2020), available at
http://www.flrules.org/Gateway/reference.asp?No=Ref-13425,
hereby adopted and incorporated by reference, and can be obtained at
Rulemaking Authority 456.013, 466.004 FS. Law Implemented 456.013, 456.048, 456.0635, 466.006, 466.0067 FS.
