(2) Copies of all application forms can be

Florida 32399-3257 or from the web at

The application must be accompanied by the appropriate application fee required
by Rules 64B3-9.001 and
64B3-9.002, F.A.C.
(3) After one year from the date of the
original submission of an application, a new application and fee shall be
required from any applicant who desires to be considered for
licensure.
(4) With regard to
persons who have been issued a social security number by the Federal
Government, disclosure of a social security number is mandatory pursuant to
Section 456.013(1)(a),
F.S., and social security numbers are used to allow efficient screening of
applicants and licensees by a Title IV-D child support agency to assure
compliance with child support obligations.
