Chapter 64B23-2 - QUALIFICATIONS FOR LICENSURE

(1) Applicants for licensure shall submit Form DH-MQA 1274 (7/2020), Application for Licensure as a Medical Physicist, incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-13208; and official documentation of board certification and licensure verification sent directly from the agency that issued the license must also be submitted.

(2) Applicants applying for a temporary license shall submit Form DH5020-MQA (6/2020), Application for Medical Physicist Temporary License, incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-13209. Applicants must designate a supervisor who is a medical physicist licensed in the same specialty as the applicant's completed residency and must have satisfactorily completed a two-year, accredited residency program. Official documentation of the completed residency program sent directly from the program must also be submitted. A temporary license shall be effective for one year from the date of issuance and may be renewed once for an additional year.
