Section 64B2-11.012 - Application for Chiropractic Acupuncture Certification

(1) Any person licensed or applying for licensure by the Board who also desires to be certified in acupuncture shall apply to the Department of Health. Application shall be made on board approved form DH-MQA 1151, (Rev 06/20), Application for Chiropractic Acupuncture Certification which is hereby incorporated by reference, and may be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-13149 or the Board of Chiropractic Medicine, 4052 Bald Cypress Way, Bin #C07, Tallahassee, Florida 32399-3257. Applicants may apply on online at http://floridaschiropracticmedicine.gov/licensing.

(2) The Board shall certify as eligible to take the acupuncture examination only those applicants who have completed a course of no less than 100 hours in acupuncture in an institution which is recognized by an accrediting agency approved by the U.S. Department of Education.

(3) The applicant is responsible for payment of the appropriate application and licensure fees.
