Section 64B2-10.0055 - Notice to the Department of Mailing Address and Place of Practice of Licensee

(1) Each licensee shall provide written notification by mail or by electronic means with confirmation to the Department notice of the licensee's current mailing address and place of practice. The term "place of practice" means the primary physical location where the chiropractor practices the profession of chiropractic.

(2) Each licensee shall provide written notification by mail or by electronic means with confirmation to the Department of a change of address within 30 days.
