On July 1 of each year, each Residency Program Director shall provide the Board with information regarding each podiatric resident using the Podiatric Resident Hospital Program Report Form DH-MQA 1140 (revised 10/19), hereby adopted and incorporated by reference, that can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-12450, the Board of Podiatric Medicine's website at http://floridaspodiatricmedicine.gov/licensing/.
