Section 64B15-10.002 - Application and Licensure Fees

(1) The application fee for an osteopathic physician license shall be $200. This fee is nonrefundable.
(2) The application fee for a limited license shall be $100. This fee is nonrefundable.
(3) The application fee for a temporary certificate to practice in an area of critical need shall be $300.00. This fee is nonrefundable. The initial licensure fee shall be $429.00. If, however, the person applying for a temporary certificate to practice in an area of critical need submits an affidavit from the employing agency or institution stating that the applicant will not receive monetary compensation for any service involving the practice of medicine, the application and licensure fees shall be waived.
(4) The initial licensure fee for temporary certificates for active duty military and veterans practicing in areas of critical need shall be $429.00. If, however, the person applying for a temporary certificate for active duty military and veterans practicing in areas of critical need submits an affidavit from the employing agency or institution stating that the applicant will not receive monetary compensation for any service involving the practice of medicine, the licensure fee shall be waived.
(5) The initial license fee paid upon submission of the application for licensure as an osteopathic physician shall be $300.
(6) The application fee for an osteopathic faculty certificate shall be $400.00.
(7) Physician assistant fees shall be those set out in Rule 64B15-6.013, F.A.C.
