Section 64B14-4.115 - Requirements for Renewal of Residency

To be eligible for a renewal of residency, the applicant must submit a renewal application to the department no earlier than 150 days prior to the expiration of the active residency. Each independent registration or dual registration is valid for 2 years after from the date of initial issuance. The renewal application must be received by the Department while the initial resident registration is still active and valid. The application titled "Renewal of Residency Registration" is form DH-MQA 1268, 07/2020, herein incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-12637, and the Department's website http://floridaorthotistsprosthetists.gov/. The registration renewal fee is set in Rule 64B14-2.006, F.A.C., and must be included with the completed application.
