Section 64B13-6.001 - Fees

The following fees are prescribed by the Board:

(1) The initial licensure fee shall be $300.00.

(2) The biennial licensure renewal fee shall be $300.00.

(3) The fee to be paid for the renewal or reactivation of an inactive license shall be $300.00.

(4) A licensee shall pay a change of status fee of $300 when the licensee applies for a change in license status at any other time than during licensure renewal.

(5) The application fee for certification as a certified optometrist shall be $250.00.

(6) The application fee for a faculty certificate shall be $100.00.

(7) The initial license fee for a faculty certificate shall be $100.00.

(8) The renewal fee for a faculty certificate shall be $100.00.

(9) The fee for a delinquent status licensee applying for active or inactive status shall be $300.00.

(10) The fee to be paid for the laws and rules examination for licensees who either have to reactivate their license or are required pursuant to paragraph 64B13-7.005(1)(h), F.A.C., is one hundred dollars ($100).

(11) The initial fee for any entity seeking approval to provide continuing education courses or programs shall be $25.00.

(12) The biennial renewal fee for any entity seeking approval to provide continuing education courses or programs shall be $25.00.

(13) The retired-status fee is $50.00.

(14) The fee for initial registration as a dispensing practitioner is $100.00.

(15) The fee for renewal of dispensing practitioner registration is $100.00 biennially.
