Section 4602 - Record Keeping for the End of Life Option Act

(a) The Department of State Hospitals will request a copy of all forms required by Health and Safety Code section 443 et seq., from the contracted entity, to put into the patient's medical file.

(b) The superior court order directing the Department of State Hospitals to facilitate the patient's participation in activities under the End of Life Option Act will be kept in the patient's medical file.

(1) If access to participation in activities under the End of Life Option Act is provided pursuant to section 4601, subdivision (c)(1), a copy of all forms required by Health and Safety Code section 443 et seq. shall be placed in the patient's medical file.

1. New section filed 6-27-2016 as an emergency; operative 6-27-2016 (Register 2016, No. 27). A Certificate of Compliance must be transmitted to OAL by 12-27-2016 or emergency language will be repealed by operation of law on the following day.
2. Repealed by operation of Government Code section 11346.1(g) (Register 2017, No. 6).
