Section 12112 - Initial License Applications; Required Forms

A person applying for Commission approval must submit the following to the Bureau:
(a) A completed Application for Employee Category License, CGCC-CH2-04 (Rev. 11/21) or Application for Owner Category License, CGCC-CH2-05 (Rev. 04/23), which are attached in Appendix A to this chapter, any applicable fees required in Section 12090, and the applicable background investigation deposit required by Title 11, CCR, Section 2037.
(b) Any applicable completed supplemental information forms, all of which are attached in Appendix A to this chapter:
(c) An Authorization to Release Information, CGCC-CH2-13 (New 05/20), which is attached in Appendix A to this chapter.
(d) One of the following:
(e) An Appointment of Designated Agent, CGCC-CH1-04.
(f) If the application is an Application for Owner Category License, CGCC-CH2-05, and the applicant is a natural person, then a completed copy of the Spousal Information, CGCC-CH2-12 (Rev. 07/22), which is attached in Appendix A to this chapter.

Amendment of subsections (a) and (b)(1)-(2) refiled 12-31-2020 as an emergency;
operative 1-1-2021 pursuant to Government Code section
11346.1(d).
Emergency expiration extended 60 days (Executive Order N-40-20) plus an
additional 60 days (Executive Order N-71-20) (Register 2021, No. 1). A
Certificate of Compliance must be transmitted to OAL by 8-2-2021 or emergency
language will be repealed by operation of law on the following
day.
3. Amendment of subsections (a) and (b)(1)-(2) refiled
6-14-2021 as an emergency; operative 6-14-2021. Expiration date of emergency
extended 60 days (Executive Order N-40-20) plus an additional 60 days
(Executive Order N-71-20) (Register 2021, No. 25). A Certificate of Compliance
must be transmitted to OAL by 1-11-2022 or emergency language will be repealed
by operation of law on the following day.
4. Amendment of subsection
(a) filed 10-7-2021 as an emergency; operative 10-7-2021 (Register 2021, No.
41). A Certificate of Compliance must be transmitted to OAL by 4-5-2022 or
emergency language will be repealed by operation of law on the following
day.
5. Amendment of subsections (a), (b)(1)-(5), (b)(7)-(9) and
(d)(1) filed 10-18-2021; operative 1-1-2022 (Register 2021, No. 43). Filing
deadline specified in Government Code section
11349.3(a)
extended 60 calendar days pursuant to Executive Order N-40-20.

Reinstatement of section as it existed prior to 12-12-2020 emergency amendment
by operation of Government Code section

7. Change without regulatory effect amending
subsections (b)-(b)(2) and (b)(4) filed 1-12-2022 pursuant to section
100, title 1, California Code of
Regulations (Register 2022, No. 2).
8. Amendment of subsection (a)
refiled 4-4-2022 as an emergency; operative 4-4-2022 (Register 2022, No. 14). A
Certificate of Compliance must be transmitted to OAL by 7-5-2022 or emergency
language will be repealed by operation of law on the following
day.
9. Amendment of subsection (a) refiled 7-5-2022 as an
emergency; operative 7-5-2022 (Register 2022, No. 27). A Certificate of
Compliance must be transmitted to OAL by 10-3-2022 or emergency language will
be repealed by operation of law on the following day.

Certificate of Compliance must be transmitted to OAL by 12-29-2022 or emergency
language will be repealed by operation of law on the following
day.
12. Amendment of subsection (a) refiled 12-23-2022 as an
emergency; operative 12-30-2022 (Register 2022, No. 51). A Certificate of
Compliance must be transmitted to OAL by 3-30-2023 or emergency language will
be repealed by operation of law on the following day.

Certificate of Compliance as to 12-23-2022 order, including nonsubstantive
amendment of subsection (a), transmitted to OAL 3-6-2023 and filed 4-14-2023;
