Section 12108 - Replacement of a Badge

(a) The Bureau will provide a replacement badge to a licensee if all of the following conditions are met:

(1) The requestor has a current valid initial, renewal, temporary, or interim license or Commission work permit.

(2) A completed Badge Replacement Request, CGCC-CH2-01 (Rev. 11/21), which is attached in Appendix A to this chapter, and the badge replacement fee specified in Section 12090, is submitted.

(b) The Bureau must notify a requestor in writing within five business days of receipt of a request, if a request or resubmitted request is deficient and identify what specific additional information is required.

(c) A replacement badge will be provided within 10 business days after the completed request is received.

(d) A replacement badge provided pursuant to this section will only be valid until the expiration date of the current license or Commission work permit.

(e) Upon the receipt of a replacement badge, the previous badge becomes invalid and may not be used thereafter.

2. Amendment of subsection (a)(2) filed 10-7-2021 as an emergency; operative 10-7-2021 (Register 2021, No. 41). A Certificate of Compliance must be transmitted to OAL by 4-5-2022 or emergency language will be repealed by operation of law on the following day.
3. Amendment of subsection (a)(2) refiled 4-4-2022 as an emergency; operative 4-4-2022 (Register 2022, No. 14). A Certificate of Compliance must be transmitted to OAL by 7-5-2022 or emergency language will be repealed by operation of law on the following day.
4. Amendment of subsection (a)(2) refiled 7-5-2022 as an emergency; operative 7-5-2022 (Register 2022, No. 27). A Certificate of Compliance must be transmitted to OAL by 10-3-2022 or emergency language will be repealed by operation of law on the following day.
5. Certificate of Compliance as to 7-5-2022 order, including further amendment of subsection (a)(2), transmitted to OAL 7-21-2022 and filed 9-1-2022; amendments effective 9-1-2022 pursuant to Government Code section 11343.4(b)(3) (Register 2022, No. 35).
