Section 1399.481 - Criteria for Provider Approval

(a) In order to be a provider, a person, organization, school, or other entity seeking approval shall submit to the Board a Continuing Education Provider Application, (Rev. 5/08), which is hereby incorporated by reference, accompanied by the fee set forth in Section 1399.462. All provider applications and documentation submitted to the Board shall be typewritten and in English.
(b) The approval of the provider shall expire two (2) years after it is issued by the Board and may be renewed upon the filing of the required application and fee.
