(a) An incarcerated person may request to withdraw from the Religious Personal Ethical Diet (RPED) Program by signing the CDCR Form 3030-D (Rev. 07/24), Religious Personal Ethical Diet Cancellation Request, which is incorporated by reference. The incarcerated person shall submit the completed CDCR Form 3030-D to any Chaplain, Community Resource Manager, or Religious Review Committee designee.

(b) An incarcerated person currently participating in an RPED may request to change to a different RPED program by submitting the applicable CDCR Forms 3030-V or 3030-R. The request shall be subject to the same application process as a new RPED diet request. No more than one RPED diet change within six months shall be permitted.
