INSTRUCTIONS

Scrap Value Sales Survey Forms shall be submitted once a month, no later than the 10th day of the following month, e.g. total transactions for the month of November should be submitted no later than December 10th. Include all information concerning glass, PET plastic and bimetal beverage container material types your company sold to any nonaffiliated container manufacturer, beverage manufacturer, processor or willing purchaser.

Combine the total weight sold, the total scrap value payment received (including any allowances given to your company), and your total shipping costs for all transactions pertaining to a specific material type, e.g. glass.

Note: Please submit a completed Scrap Value Sales Survey Form every month covering all material types for which you are certified, regardless of whether or not you sold a given commodity. For example, report zero tonnage for any and all commodities you did not sell to any nonaffiliated container manufacturer, beverage manufacturer, processor or willing purchaser.

COMMODITY

Glass- All certified processors must report the monthly total weight, in tons, of postfilled beverage container material types (CRV and non-CRV), including the monthly weight, in tons, by color, sold to any nonaffiliated container manufacturer, beverage manufacturer, processor or willing purchaser.

Plastic- Plastic reported should include polyethylene terephthalate (PET) ONLY. Processors shall differentiate between California Redemption Value (CRV) PET and non-CRV PET.

Plastic Recycling Corporation of California (PRCC) members and non-PRCC members shall report volumes and payment information the month materials are shipped.

Bimetal- Beverage containers which consist of one or more metals and which are composed primarily of steel. Bimetal reports should not include non-beverage container metals.

TOTAL WEIGHT SOLD

Total net weight (in tons) of beverage container material types sold. Container material types for purposes of reporting monthly scrap value, are any recyclable container including beverage, food and/or drink packaging material, other non-redeemable containers, out-of-state beverage containers, line breakage or rejected containers, of the same material composition as redeemable containers covered by the California Beverage Container Recycling and Litter Reduction Act. Plastic weight reported should only include polyethylene terephthalate (PET). Identify and report California Refund Value (CRV) PET and Non-CRV PET transactions in separate boxes.

TOTAL SCRAP VALUE PAYMENT RECEIVED

The scrap value amount received from any nonaffiliated container manufacturer, beverage manufacturer, processor or willing purchaser including any additions (e.g. freight allowances, service fees) pertinent to the specific sales transactions for that particular month.

TOTAL SHIPPING COST

Enter the total cost to you for delivery to any nonaffiliated container manufacturer, beverage manufacturer, processor or willing purchaser's facility. If the purchaser pays for shipping of the sold materials, enter zero, (0).
