(B) New Restricted Species Permit Application (FG 1312 (Rev. 3/12)), incorporated by reference herein.
1. Any person applying for all permits specified in subsections 671.1(b)(1)-(12), except for the permit specified in subsection 671.1(b)(7) (Native Species Exhibiting Permit), shall submit this application when applying for a new permit.

(C) Native Species Exhibiting Permit Amendment Request form (FG 1312a (Rev. 3/12)), incorporated by reference herein.
1. Any person with a valid Native Species Exhibiting Permit specified in subsection 671.1(b)(7) and requests to add species, change authorizations and conditions or change the facility's name and/or address shall submit this form.

(D) New Native Species Exhibiting Permit Application (FG 1312b (Rev. 3/12)), incorporated by reference herein.
1. Any person applying for the Native Species Exhibiting Permit specified in subsection 671.1(b)(7) shall submit this application when applying for a new permit.

(E) Restricted Species Permit Inventory of Animals form (FG 1313 (Rev. 3/12)), incorporated by reference herein.
1. Any person applying for any permits specified in subsections 671.1(b)(1)-(12), except for the permit specified in subsection 671.1(b)(7) (Native Species Exhibiting Permit), shall submit this form when applying for a new permit.

1. Any person applying for the Native Species Exhibiting Permit specified in subsection 671.1(b)(7) shall submit this form when applying for a new permit.

(G) Restricted Species Permit Amendment Request form (FG 1313b (Rev. 3/12)), incorporated by reference herein.
1. Any person with a valid permit specified in subsections 671.1(b)(1)-(12), except for the permit specified in subsection 671.1(b)(7) (Native Species Exhibiting Permit), and requests to add species, change authorizations and conditions or change the facility's name and/or address shall submit this form.

(H) Restricted Species Nonresident Exhibiting Permit Itinerary form (FG 1316 (New 11/10)), incorporated by reference herein.
1. Any person applying for a Nonresident Exhibiting Permit specified in subsection 671.1(b)(6) or amending an existing itinerary on file with the department shall submit this form as required in subsection 671.1(c)(3)(F).

(I) Any person renewing an existing restricted species permit specified in Section 671.1(b), shall submit their request using an annual report generated and provided by the department.

(2) Commercial Permit for Native Rattlesnakes

(3) Determination that a Transgenic Aquatic Animal is not Detrimental
(A) The applicant shall apply in the form of a letter, on letterhead if an entity, for a department determination that a transgenic aquatic animal is not detrimental in accordance with Section 1.92 and shall include all of the following:
1. The name, mailing address, telephone number(s), and e-mail address of the person seeking to import, possess, distribute, and sell the transgenic aquatic animal or of the principal contact person if an entity seeks to import, possess, distribute, and sell the transgenic aquatic animal.

2. A detailed analysis based on credible science containing:
a. The common and scientific names of the species for which an exemption is sought.

b. A description of the life history of the species.

c. A description of the method(s) by which the genome of the species has been deliberately altered, modified, or engineered.

d. The known or anticipated effects of the genetic alteration, modification or engineering of the species.

e. An analysis of the potential risk to native fish, wildlife, or plants posed by the presence of the transgenic aquatic animal within California.

f. A description of the applicant's proposed importation, possession, distribution, and sale of the transgenic aquatic animal within California.

3. Certification in the following language: I certify that the information submitted in this application is complete and accurate to the best of my knowledge and belief. I understand that any false statement herein may subject the application to rejection, or the department determination to revocation, and to civil and criminal penalties under the laws of the State of California.
a. The original signature of the person, or principal contact person if an entity, seeking the determination.

4. The applicant shall submit a separate application and nonrefundable fee of $4,790 per species of transgenic aquatic animal.

5. The applicant shall submit one paper copy, and an electronic copy (via email or other device as directed by department staff) containing all application materials, and the application fee, to the Fisheries Branch Chief at 830 S Street, Sacramento, CA 95811.

(B) Contents of the Department Determination
1. The department shall issue a determination in writing, based on the information provided by the applicant, and any other relevant credible scientific information in the possession of the department or submitted to the department.

2. The determination shall state whether:
a. The presence of the transgenic aquatic animal within California is detrimental and subject to regulation under Section 671 and subsection 671.1(a)(8); or,

b. The presence of the transgenic aquatic animal within California is not detrimental and poses no reasonably foreseeable risk to native fish, wildlife, or plants and is not subject to regulation under Section 671 and subsection 671.1(a)(8).

c. In making its determination, the department may impose reasonable conditions to ensure the proposed importation, possession, distribution, and sale of the transgenic aquatic animal within California is not detrimental to native fish, wildlife, or plants.

d. The department may revoke or change its determination at any time upon newly-obtained information or circumstances involving said animal's detrimental impacts.

3. If the department identifies deficiencies in the application, requiring additional time or further review, the department shall reject the application and provide written notification of the identified deficiencies in the application to the applicant. No additional fee is required if the application, with required information, is resubmitted within one year of receipt of the original application.

(C) Effect of Department Determination
1. Once it receives a determination from the department that the transgenic aquatic animal poses no reasonably foreseeable risk to native fish, wildlife, or plants, the applicant or its authorized agent may import, possess, distribute, and sell the animal within the state provided that both the applicant and its authorized agent possess and provide within three business days, upon request by the department, a copy of the department's determination.

2. Any wholesaler or retailer purchasing a transgenic aquatic animal from the applicant or its authorized agent may import, possess, distribute, and sell the animal provided that the wholesaler or retailer possesses and provides within three business days, upon request by the department, both a copy of the department's determination and written documentation to demonstrate that the animal that the wholesaler or retailer purchased originated from the applicant or its authorized agent.

3. Individuals purchasing a transgenic aquatic animal that originated from the applicant, its authorized agent, or wholesalers or retailers as authorized by this section may possess the animal, without a copy of the department's determination or any other documentation, provided that the animal is maintained in a closed system and not placed in the waters of the state.

(B) Falconry License Renewal Application (FG 360 (New 2/13)) incorporated by reference herein.
1. An applicant for a license renewal that has not lapsed as specified in subsection 670(e) shall submit a License Renewal Application with the appropriate fee(s) listed in paragraph (b)(1)(A).

(C) New Falconry License Application (FG 360b (New 2/13)) incorporated by reference herein.
1. An applicant for a new license as specified in subsection 670(e) shall submit a New Falconry License Application with the appropriate fee(s) listed in paragraph (b)(1)(A).

2. An applicant for a lapsed license renewal as specified in subsection 670(e) shall submit a New Falconry License Application with the appropriate fee(s) listed in Paragraph (b)(1)(A).

(D) Apprentice Falconer's Annual Progress Report (FG 360c (New 2/13)) incorporated by reference herein.
1. A licensed Apprentice falconer who is applying for license renewal, or within 10 calendar days after expiration of the license, whichever comes first, shall submit an Apprentice Falconer's Annual Progress Report as required in subsection 670(f).

(E) Raptor Facilities and Falconry Equipment Inspection Report (FG 360d (New 2/13)) incorporated by reference herein.
1. A new applicant, an applicant renewing a lapsed license, or a licensee that moves housing facilities to a new address, shall initiate an inspection by submitting a Raptor Facilities and Falconry Equipment Inspection Report as required in subsection 670(j) with the specified Inspection Fee(s) listed in paragraph (b)(1)(A).

(F) Resident Falconer Raptor Capture Recapture and Release Report (FG 360f (New 2/13)) incorporated by reference herein.
1. A licensee shall submit a Resident Falconer Raptor Capture Recapture and Release Report to report a capture, recapture or release of a raptor as required in subsection 670(f).

(G) Falconry Hunting Take Report (FG 360h (New 2/13)) incorporated by reference herein.
1. An applicant for license renewal or within 10 calendar days after expiration of the license, whichever comes first, shall submit a Falconry Hunting Take Report as required in subsection 670(f).

(H) Special Raptor Capture Drawing Application (FG 360i (New 2/13)) incorporated by reference herein.
1. Any resident or nonresident possessing a valid General or Master falconry license may submit a Special Raptor Capture Drawing Application with the appropriate fee(s) listed in paragraph (b)(1)(A) to be entered into a drawing for a Special Raptor Capture Permit as required in subsection 670(g) for the Northern goshawk or prairie falcon.

(I) Nonresident Falconer Application for Raptor Capture Permit (FG 361 (New-2/13)) incorporated by reference herein.
1. A nonresident licensed falconer applying to capture a raptor, excluding species with capture quotas, as specified in subsection 670(g) shall submit a Nonresident Falconer Application for Raptor Capture Permit with the appropriate fee(s) listed in paragraph (b)(1)(A).

(J) Nonresident Falconer Raptor Capture Permit and Report (FG 361a (New -2/13)) incorporated by reference herein.
1. A nonresident licensed falconer shall submit a Nonresident Falconer Raptor Capture Permit and Report, whether successful or unsuccessful in capturing a raptor, as required in subsection 670(g).

(c) Applications, Forms and Fees for multi-year permits valid from date of issuance.

(1) Scientific Collecting Permits (SCPs)
(A) Application and Permit Fees

The application fee and amendment fee is non-refundable for all permit categories and types. Application and amendment fees shall be submitted with an application, and the permit fee is due within 60 calendar days upon after notice of permit approval, or the Department may refuse to issue the permit and change the approval into a denial. The application fee is required, should an applicant re-apply. All fees are subject to Section 713 of Fish and Game Code, and may be adjusted to include other fees required by license agents, pursuant to Fish and Game Code Section 1055.

1. General Use application and permit fees are required per the department review program as outlined in Title 14, Section 650(d), CCR. Specific Use application and permit fees include all department review programs.
