Section 182.01 - Application for Disabled Person Placard or Plates Form

Applicants for a disabled person placard or plates shall complete and submit an Application for Disabled Person Placard or Plates form (REG 195, REV. 4/2018), which is hereby incorporated by reference. This form is available on the department's internet website at www.dmv.ca.gov, in any field office, or by calling the department's Telephone Service Center at (800) 777-0133.

(a) The application may be completed and signed by a parent or guardian of a disabled minor if parts G and H of a Statement of Facts form (REG 256, REV. 8/2008), which is hereby incorporated by reference, are completed and signed by a parent or guardian indicating his or her relationship to the minor. This form is available on the department's website at www.dmv.ca.gov, in any field office or by calling the department's Telephone Service Center at (800) 777-0133.

(b) A person, other than the applicant, may complete the application if the applicant's disability prevents him or her from completing the application. Parts G and H of a REG 256 must be completed and signed indicating relationship to the applicant.

(c) The Doctor's Certification of Disability is not required under the following circumstances:

(1) The applicant indicates that a disabled person or disabled veteran license plate or placard have previously been issued unless the placard was cancelled by the department or if the placard is no longer on record with the department.

(2) The applicant appears in person and has a readily observable and uncontested permanent disability as specified in section 182.00(d) and the disability is verified by an employee of the Department of Motor Vehicles.

(3) The application is being submitted on behalf of an organization or government agency involved in the transportation of disabled persons or disabled veterans.
