Section 125.20 - Notice of Removal

(a) An installer shall submit to the department within three working days a form DL 922 when the installer removes an ignition interlock device.

(b) A manufacturer shall account for each form DL 922 issued to the manufacturer. If any unused forms become lost, stolen, or misplaced the manufacturer shall immediately notify local law enforcement and provide written notification to the department.
