Section 2209 - Applications
Applications and enrollment forms which are to be incorporated in documents which must be submitted to the Commissioner shall be subject to the same filing requirements as the documents in which they are to be incorporated, except as otherwise provided. Where a previously approved application is to be incorporated in a newly submitted document, the cover letter shall state the California Department of Insurance file number for the application, the application form number, and the date of the Commissioner's action upon it.
