Section 2699.6823 - Family Contribution Sponsor Certification Requirement

(a) Any person or entity seeking to provide payment as a family contribution sponsor must certify both of the following:
(1) that the person or entity is not ineligible to
be a family contribution sponsor under Section
2699.6821, and
(2) that the person or entity acknowledges that
the Board has taken no position as to whether payment of premiums as a family
contribution sponsor by any person or entity would be in violation of federal fraud
and abuse laws.
(b) The certification shall be made on the Healthy Family Program Family Contribution Registration form (HFP-Sponsor 1 (new 7/00)).
