Regulation No. 5 is hereby amended by adding the following new paragraph E:
(E) Any Lessor Employing Unit will provide individual, written notice to each employee specifying the appropriate Employment Security Department account number and employer name to be used for the purpose of applying for unemployment insurance benefits.
This amended regulation shall take effect and be in full force on and after July 1, 2003.
