Section R4-33-211 - Notice of Appointment

A. An administrator shall provide written notice to the Board, within 30 days, of being appointed administrator of a nursing care institution or terminating an appointment.
B. An administrator shall include the following, as applicable, in a notice regarding the administrator's appointment:
1.
Administrator's name,
2.
Administrator's license number,
3.
Name and address of the nursing care institution to which the administrator is
appointed,
4. Date of
appointment,
5. Name and address of
the nursing care institution at which the administrator's appointment is
terminated, and
6. Date of
termination.
