Section R4-2-206 - Form and Content of Authorizations

A. An individual can show interest by completing an authorization card or signing a petition.
B. An individual authorization card submitted to the Board as evidence of a showing of interest is not valid unless the card contains only one name, one signature, and the following legible information:
1. The employee's name, name of employer, and
social security or employee identification number;
2. The signature of the employee and date in
the employee's own handwriting; and
3. A statement that the employee is knowingly
providing authorization that the petitioner may represent that employee for the
purpose of collective bargaining in Arizona only, and authorization that the
petitioner may file a petition for election under A.R.S. §
23-1389.
C. A signature petition submitted as authorization to evidence a showing of interest is not valid unless it contains:
1. The signature of the employee,
social security or employee identification number, and date in the employee's
own handwriting; and
2. The name of
the employer and a statement that the employee is knowingly providing
authorization that the petitioner may represent that employee for the purpose
of collective bargaining in Arizona only, and authorization that the petitioner
may file a petition for election under A.R.S. §
23-1389.
