(a) To be eligible for payment under this chapter, a community behavioral health services provider or a mental health physician clinic must maintain, for each recipient served, a clinical record in accordance with 7 AAC 105.230 that also must include a
(1) written report that documents the results
of a professional behavioral health assessment conducted in accordance with
7
AAC 135.110 that also identifies the
(A) recipient's presenting problems,
diagnosed conditions, and functional deficits that require treatment;
and
(B) treatment recommendations
for the recipient's presenting problems, diagnosed conditions, and functional
impairments; and
(2)
behavioral health treatment plan that meets the requirements of
7
AAC 135.120 and includes
(A) the recipient's identifying
information;
(B) the date
implementation of the behavioral health treatment plan will begin;
(C) treatment goals based on the presenting
problems, assessed conditions, and functional deficits that are the current
focus of treatment; and
(D) the
services that will be used to address the written goals.
(b) The clinical record must include all the changes made to a recipient's behavioral health treatment plan and updates to the professional behavioral health assessment.
