7 AAC 128.010 - Long-term services and supports targeted case management

(a) The services provided under this section must assist and enable an individual eligible under 7 AAC 125.010 - 7 AAC 125.199, 7 AAC 127, or 7 AAC 130 to access necessary medical, social, educational, developmental, and related services,

(b) The department will pay for the long-term services and supports targeted case management services as specified in 7 AAC 145.290 for

(1) the following, prepared and submitted in a format provided by the department:
(A) one initial application for home and community-based waiver services or Community First Choice services; the department may approve one additional application for an individual who does not qualify for services based on the individual's first application;

(B) one initial support plan for home and community-based waiver services and Community First Choice services;

(C) annual renewal support plans for home and community-based waiver services and Community First Choice services; and

(2) case management for Community First Choice services only if for the month services are billed,
(A) the recipient's care coordinator prepared and submitted a support plan amendment in a format provided by the department;

(B) the recipient's care coordinator was in contact with the recipient on occasions agreed to by the recipient, and the time and frequency of the contacts are indicated in the recipient's support plan; or

(C) when contact with the recipient's care coordinator was initiated by the following individuals:
(i) the recipient or the recipient's representative;

(ii) members of the team responsible for developing the recipient's support other than the recipient's care coordinator;

(iii) provider agencies or individuals who provide formal or informal care and supports for the recipient;

(iv) the department, if the contact results in activity required of the recipient's care coordinator.

(c) The documents submitted for both initial and renewal support plans under this section must

(1) indicate the individual's choice of service as described under 7 AAC 130.218(c)(5);

(2) comply with 7 AAC 127.039, if the support plan specifics Community First Choice services and other long-term services and supports under 7 AAC 127.039(a)(3); and

(3) comply with the provisions of 7 AAC 130.217 and 7 AAC 130.218, if the support plan specifies home and community-based waiver services under 7 AAC 130.

(d) To be eligible to receive payment for services under this section, a provider must be

(1) certified under and comply with the requirements of 7 AAC 130.220 and 7 AAC 130.238; and

(2) enrolled as a provider of care coordination services in accordance with 7 AAC 105.210.
