7 AAC 127.145 - Reporting recipient changes

(a) A recipient shall report to each of the recipient's Community First Choice services providers and the recipient's care coordinator any of the following changes by telephone, by facsimile transmission, by electronic mail, in writing, or in person not later than 15 days after the recipient knows of the change:

(1) a change in the recipient's
(A) place of residence or living an-arrangement;

(B) personal contact information;

(C) representative;

(D) medical provider; or

(E) marital status;

(2) a change to the recipient's eligibility to receive home and community-based waiver services under 7 AAC 130, including a change to the recipient's support plan under 7 AAC 130.217. or any other changes lo health care services that may duplicate the recipient's Community First Choice personal care services;

(3) an improvement or decline in the recipient's physical condition;

(4) a change in household composition or in the number of other recipients receiving services in the recipient's residence.

(b) Not later than ! 5 days after the Community First Choice services provider agency learns of the change, the provider agency shall report, in a format provided by the department, any change that could affect the recipient's level-of-care assessment, service level authorization. or level of services provided lo the recipient. The provider agency may submit the report by facsimile transmission, by electronic mail, or in writing.

(c) Upon review of a report under (b) of this section, the department may conduct a new assessment or interim level-of-care review of a recipient if the department determines that a new assessment or interim level-of-care review is necessary to confirm the recipient's eligibility for Community First Choice services, need for services at the level of assistance currently approved for the recipient, or need for the activities specified in recipient's current service level authorization.
