An application for ATAP benefits must be signed by

(1) a caretaker relative of the dependent child who is the basis of the application for ATAP benefits, as determined under 7 AAC 45.225;

(2) both parents of the dependent child, if the child is residing with both parents;

(3) an individual appointed under 7 AAC 45.157(a) as the authorized representative of a caretaker relative; or

(4) an individual acting responsibly in the place of the caretaker relative, as determined under 7 AAC 45.157(b), if

(A) the caretaker relative under (1) of this section is incompetent or incapacitated; or

(B) both parents under (2) of this section are incompetent or incapacitated.
