To receive Alaska Temporary Assistance Program (ATAP) benefits under AS 47.27, an applicant must apply for benefits, in writing or by electronic means, on a form provided by the department. The application form must clearly identify the applicant by including a legible name, mailing address, and signature or witnessed mark, and, as required by AS 47.27.020(b), must include the attestation regarding other state assistance. The application form must be filed with a district office of the division handling public assistance matters in the department, or a fee agent or other division designee.
