A participating provider shall retain records required for participation and compliance with the child care assistance program under this chapter for at least three years from the date of the record's creation. The provider shall make available for inspection during normal business hours, upon request of the department or designee, records including

(1) each license or permit and each approval required for operation;

(2) copies of rates and responsibilities forms;

(3) provider's requests for payment;

(4) daily attendance records that reflect the date and time children are in care, on a form provided or in an manner approved by the department;

(5) children's records including emergency information for each child on a form prescribed by the department;

(6) caregiver employment records; and

(7) documentation of emergency evacuation drills.
