7 AAC 18.355 - Report to former employees

(a) A registrant, at the request of an individual formerly employed or associated with the registrant, shall furnish to the individual a report of the individual's exposure to ionizing radiation as shown in records maintained under 7 AAC 18.340. The registrant shall provide the report to the individual requesting the report within 30 days from the date the request is received by the registrant. The report must cover each calendar year of the individual's employment or association involving exposure to ionizing radiation, or a shorter period requested by the individual. The report must be in writing and contain the following statement: "This report is furnished to you under the regulations of the Department of Health and Social Services regulations. You should preserve this report for future reference."

(b) The registrant may require the requesting individual to provide a government-issued unique identifying code and the dates and locations of employment or association.
