(a) A facility shall develop procedures necessary to evaluate the program and shall conduct an evaluation annually.
(b) The evaluation must include
(1) information about demographic
characteristics;
(2) assessment of
community and client reaction to services, which may include questionnaires,
surveys, or board reports; and
(3)
progress report for the five-year plan for increasing
self-sufficiency.
