In its grant application, a facility shall include a five-year plan for increasing self-sufficiency. A five-year plan for self-sufficiency must include
(1) a summary, by source and amount of revenues and expenses, that the facility expects during each of the five years starting with the current state fiscal year;
(2) an estimate of revenue-sharing under AS 29.89.030, and other federal and state support that the facility could receive during each of the five years; and
(3) an estimate of current taxable property values in the area or areas that the facility would serve under the grant.
