If a plan of care requires the consultation or direct services of a dietitian, the home health agency must ensure that those services are provided by a dietitian registered by the Commission on Dietetic Registration of the American Dietetic Association. A dietitian providing services for a home health agency shall
(1) review the dietary assessment of a patient with special dietary needs, and monitor the management of any special diet requirements;
(2) participate in developing a plan of care regarding a patient's dietary needs, including dietary goals, objectives, and interventions;
(3) provide written directions to family and staff for meal planning for a special diet;
(4) participate in patient care conferences;
(5) participate in educating the patient and family about the patient's dietary needs and management;
(6) participate in agency inservice training;
(7) prepare clinical progress notes;
(8) prepare summaries of care;
(9) provide the agency with an assessment and consultation if hyperalimentation services are being provided; and
(10) maintain liaison with the treating physician, advanced practice registered nurse, or physician assistant and with the agency staff to ensure that their efforts are coordinated effectively and support the objectives outlined in a plan of care.
