An amendment may be made only within the bureau by authorized personnel, in accordance with the instructions of the State Registrar, and upon the presentation of sufficient documentary or other evidence as required by the State Registrar. Such amended certificate or record shall be plainly marked "Amended"; and show the items amended, the date thereof, and a summary description of the evidence submitted in support thereof.
