The belated or delayed certificate forms shall be those prescribed and furnished by the bureau, and the evidentiary or other information required shall be submitted on such forms or in such manner as shall be determined by the State Registrar. Original documents may be submitted to the bureau at the owner's risk; they may be copied, abstracted, or otherwise included in the substantiating evidence, but shall be returned to the owner if so requested. The required proof or evidence needed to substantiate a delayed certificate shall be the responsibility of the person concerned, and the burden of proof shall rest with such person. Fees for registering a delayed certificate and any charges for obtaining substantiating evidence shall be as prescribed by the State Registrar, and shall be paid to the bureau by the applicant before such certificate may be registered.
