22 AAC 30.030 - Application process; qualifications

(a) An individual who wishes to become an approved sex offender treatment provider must apply to the department on a form provided by the department. Only an individual may be approved as a sex offender treatment provider.
(b) To become an approved provider under this chapter, an individual must
(1) have a current
professional license, in good standing, issued under AS 08, as a psychiatrist,
psychologist, psychological associate, social worker, marital and family
therapist, or professional counselor;
(2) be of good moral character; and
(3) agree to abide by the standards set out
in 22 AAC 30.200 in providing sex
offender treatment to a sex offender who is under the department's
jurisdiction.
(c) An application for provider approval must include
(1) the applicant's name, business mailing
address, and telephone number;
(2)
a statement of the applicant's educational degrees, the year they were
obtained, and the institutions from which they were obtained;
(3) verification from the relevant Alaska
licensing board that the applicant has a current professional license, as
described in (b) of this section, in good standing;
(4) a history of the applicant's specialized
training in the treatment of sex offenders;
(5) a history of the applicant's professional
work experience;
(6) the
applicant's complete criminal history, if any;
(7) information about any investigations of
the applicant by any licensing authority in this or any other jurisdiction for
possible professional license violations; and
(8) references from at least two individuals
familiar with the applicant's professional training and experience.
(d) Except as provided in 22 AAC 30.100, initial department approval of a provider lapses three years from the date of the approval.
