(a) An applicant must furnish information
showing a history of successful placer mining in Alaska. Examples of such
information include
(1) the dates, locations,
and descriptions of the applicant's placer mining experience in
Alaska;
(2) the number of employees
supported by the applicant's previous placer mining operations in Alaska,
listed by year;
(3) a list of
equipment used, owned, or leased in the applicant's previous placer mining
operations in Alaska;
(4) copies or
a list of permits from government agencies, or other information to verify
placer mining operations in Alaska;
(5) financial records, balance sheets, credit
references, profit-and-loss statements, loans received or paid off, or tax
information that the applicant may wish to volunteer to demonstrate success in
placer mining; and
(6) any other
information that shows evidence of experience in Alaska placer
mining.
(b) An applicant
must furnish information to show an ability to study, test, and adequately
report on the proposed new methods and to produce verifiable results. This
information may include
(1) a description of
education or experience related to water analysis, flow measurement, sampling
techniques, and similar scientific methods;
(2) association or cooperation with research
groups or government agencies to conduct studies pertinent to the management
and use of water in mining;
(3)
copies or a list of the applicant's publications, reports, papers, or
presentations at meetings or conferences concerning water pollution control,
water use reduction, waste disposal, gold recovery, or related topics;
and
(4) any other information that
would help prove ability to produce verifiable results.
(c) An applicant shall provide information
about the proposed demonstration project, including
(1) the location of the operation;
(2) the name and location of any affected
water bodies;
(3) a description of
the proposed project which tells how the project is innovative, how it differs
from similar projects, and how the proposal, if successful, will benefit placer
mining and pollution control in Alaska;
(4) sketches, flow diagrams, or blueprints of
the proposed operation;
(5) lists
and descriptions of equipment needed, and purchase price or design and
construction costs for each;
(6)
descriptions of time and labor needed to construct, install, and operate the
project;
(7) schedules for
construction, operation, study, and testing of the project;
(8) a detailed description of how the
applicant intends to study and test the project, with special attention to
sampling techniques and equipment, and the means to determine the project's
success; and
(9) a description of
the reports and final written products that will be supplied to the department
at project completion.
(d) A cover sheet or processing form
available from the department, must be submitted with each application.
Application forms are also available from the department, but their use is
optional.
