12 AAC 44.852 - Nurse aide skills checklist

(a) A certified nurse aide training program must maintain a nurse aide skills checklist that records the performance of each student. The nurse aide skills checklist must include
(1) each of the skills listed in
12 AAC 44.845 and
12 AAC 44.847;
(2) the date each skill was practiced or
demonstrated;
(3) the student's
satisfactory or unsatisfactory performance of a skill each time is was
practiced or demonstrated; and
(4)
the name and signature of the instructor who supervised the student's
performance of a skill.
(b) After a student has completed a certified nurse aide training program, the program provider shall provide a copy of the nurse aide skills checklist to the student.
