12 AAC 44.785 - Reinstatement of a revoked license

(a) One year after revocation of a license, the nurse or advanced practice registered nurse may apply to the board in writing for reinstatement.

(b) The applicant for reinstatement shall appear before the board.

(c) The board may impose restrictions upon the nurse or advanced practice registered nurse when reinstating a license.

(d) The applicant is responsible for providing a written evaluation from the employer regarding the applicant's performance and will be evaluated by the board to determine whether the license will be reinstated.

12 AAC 44.785 formerly appeared as 12 AAC 44.920. As of Register 103, the section was renumbered as 12 AAC 44.785. The history note for 12 AAC 44.785 reflects the history of the section both before and after renumbering.
